Once you have been admitted to a hospital while overseas, we will obtain regular updates on your status from the treating physicians and nurses taking care of you and send these updates to your military health care provider. We are also available to assist you directly with any medical questions or concerns you may have. Our goal is to make sure you are fully informed to make your hospital stay as comfortable as possible. A representative of our NPP Team of bilingual nurses and doctors will either visit your bedside and/or contact you and your treating physician during your stay in order to obtain medical updates from your treating medical team. Our goals include a positive patient experience, assurance that you are receiving quality health care services, and facilitating a seamless transition of care between the host-nation provider and the military clinic. <ul> <li>Your local NPP Team can help you navigate the health care system, including overcoming language barriers and ensuring that you understand the health care being delivered to you overseas. </li> <li>The Near Patient Nurse may contact or visit you while you are admitted to a hospital. They will talk to your treating medical team and medically monitor your care when you are admitted to a civilian facility overseas and ensure that your Primary Care Manager is informed about your care (where applicable). </li> <li>The Near Patient Nurse may ask about your experience with the hospital staff and can address any concerns you may have regarding the care you receive. </li> </ul> Beneficiaries seeking assistance from their local Near Patient Team can contact them through the <a href="https://tricare-overseas.com/beneficiaries/resources/beneficiary-app" target="_blank">MyCare Overseas™ App.</a> They can scan the QR code to download the app, and chat directly with a Near Patient Program Team member. Chat is available Monday through Sunday, 08:00-20:00 local time in all <a href="https://tricare-overseas.com/beneficiaries/resources/Near-Patient-Program" target="_blank">Near Patient Program (NPP)</a> locations. Chat is available to all TRICARE Prime Overseas and TRICARE Prime Remote Overseas beneficiaries enrolled in the NPP, who have had at least one authorization issued after January 1, 2020. If you are unable to access the chat, <a href="https://tricare-overseas.com/contact-us" target="_blank">please call your TOP Regional Call Center</a> on the 24/7 toll-free number.   <h4>WHAT DOES THE NPP MODEL MEAN FOR TOP PROVIDERS? </h4> TOP Providers interface with NPP Teams on a frequent basis. The NPP Team is supported by non-clinical staff. These include Medical Records Collection (MRC) Teams and the NPP Country Leads who manage relationships with TOP Providers, oversee education, and resolve non-clinical issues. The NPP Physician Leads provide medical oversight of beneficiary care and collaborate with the MTF Primary Care Manager (PCM), as needed. The Near Patient Nurses educate beneficiaries on the local health care system and provide day-to-day assistance.    The NPP leverages dedicated Clinical Quality Assurance Leads to better identify how TOP Providers can improve care and determine if any additional training or support is required. The NPP Team works closely with the TOP Provider to identify and bridge cultural gaps and nuances in order create an optimal care experience for TRICARE patients overseas. The NPP Team closely collaborates with TOP Providers to address any clinical care concerns and improve the quality of care. Through the NPP model, International SOS and TOP Providers effectively collaborate to support TOP beneficiaries, bridging cultural nuances, aligning expectations, and explaining health care services.
